Requirements for Affidavits and Reports
submitted in support of
Permit Renewals and Lease Continuations

SUBMISSIONS
Submissions must be marked "CONFIDENTIAL" and delivered to the attention of:

CONFIDENTIALITY
All reports, maps, and data submitted in support of an Affidavit to record Permit work or in support of a Section 58(3)(c) Lease continuation will be held confidential for 10 years, but may be released sooner if petroleum and natural gas tenures directly related to the Affidavit of Work or lease continuation are cancelled.
In cases where a report includes information subject to third-party confidentiality agreements it is the responsibility of the party submitting the report to obtain the necessary waiver or exception from the information vendor that will allow the information to be released in accordance with these Requirements and the Drilling and Production Regulation. |
AFFIDAVITS (Permit Renewals Only)
An affidavit is to be submitted when an application for a permit renewal is made under Section 47(2) and 47(3) of the Petroleum and Natural Gas Act. If total costs have not been compiled, and/or the related report is not complete, then the submission of the final Affidavit of Work or the required report may be delayed for a period of up to ninety days with the approval of the Director. It must not be assumed that a delinquent submission will receive approval for the purposes of work credits.
Affidavits must be notarized and include the following information:
- Total expenditures
- The nature of the work
- The dates the work was carried out
- The permit(s) the work is to be applied to
- The name of the company who performed the work
- The name(s) of the Permit holder
Ministry forms are made available for convenience only; the required information may be submitted in other formats.
REPORTS
The report submitted in support of each affidavit (Permits) or to satisfy a requirement of a Section 58(3)(c) work program (Leases) must conform to the following criteria:
A. Presentation and Identification Requirements
Each report must:
- be written and labelled in English
- include on its cover, or a separate cover page, each of the following items:
- Title and date of the report
- Dates of work commencement and completion
- Work Program Number (assigned by the Oil and Gas Commission on approval of the Application for Approval of Geophysical Exploration)
- List of Permit(s) or Lease(s) included in the project
- Name, title, and signature of the person submitting the report; and
- when an affidavit for work credit is submitted, the name, title, and signature of an authorized representative of the titleholder submitting the report, and/or
- when a geological or geophysical report is submitted, the name, title, signature, and professional seal of the registered engineer, geologist or geophysicist
B. Content Requirements
General (All Reports)
Every report must include the following:
- Table of Contents page listing subcategories and all enclosures
- Reference Map showing outlines of Permit(s) or Lease(s) included in the project and the location(s) or route(s) of the work performed
- List of project operators and contractors, including names of contact people
- Copies of all maps and sections required to make the reported interpretations
- Discussion and interpretation section

The discussion and interpretation section is considered fundamental to the report’s utility. Its omission will normally result in the rejection of the report. |
Test Hole programs
In addition to the General content requirements, include:
- Map showing the location and ground elevation of each test hole
- Detailed descriptions of all samples and cores recovered from each hole
- Copies of all mud-gas and wireline log records
- Discussion of significant occurrences of water, gravel, coal, and hydrocarbons
Geophysical Survey programs
In addition to the General content requirements, include:
- Description of equipment, recording parameters, and instrument arrangements in sufficient detail to evaluate data quality
- Statements of the total length, in kilometers, of each survey line
- All processed record sections with line number, shot point numbers, and processing labels clearly indicated. A representative number of the record sections must have key horizons interpreted and labelled, unless poor data quality does not permit this.
- Representative isochron, time-structure, amplitude, attribute or other maps, demonstrating the points made in the discussion section. Any horizon appearing in a submitted map must also be clearly identified on the record sections from which the map was generated.
Surface Geology and Photogeology programs
In addition to the General content requirements, include:
- Rationale used to design the traverse or survey
- Copies of all observation logs or photographs
C. Mapping Requirements
Each map submitted must be prepared to generally accepted drafting standards and include these elements:
- Title block
- Legend
- NTS or DLS grid labelled in a manner that allows an uncomplicated inference of surface locations and gas spacing areas
- Data values and contour intervals of mapped surfaces, horizons, or other features
