Mineral Tenure Act Forms

Note: Some of these forms have a specific purpose and cannot be generally used to register events on claims or leases in Mineral Titles Online (MTO). Refer to the specific instructions in the text that accompanies each form.

Coal Act and Mineral Tenure Act forms are available in two formats, Microsoft Word format and Adobe Acrobat format.  To view the forms in Word format you must have Word installed on your PC or have the free Microsoft Word Viewer installed on your PC.  To view the forms in Acrobat format you must have the free Acrobat viewer installed on your PC.  To learn more about Acrobat click here.
